I would recommend against it. That's become something of a cliche. Find a more interesting way to show your protagonist's mental problems.
If you're interested in realism, note that most people who have hallucinations are aware that the hallucinated object or person is not real; conversely, most people who have a delusional belief in an imaginary person do not also hallucinate that person. Somebody who does both at once isn't likely to be otherwise functional enough to live in a non-supervised environment.
