I need a new conflict.....

Okay, so I figured out how to get past my writers block but.... I'm at 60 pages and my story is wrapping up. The patient who's sick has a diagnosis and a treatment. The doctor who was distracted by a romance got his focus back and diagnosed the patient. I need something other than breaking the romance up or the patient dying to get the plot moving again..... the trouble is that I haven't been able to explain why the patient was having hallucinations about someone very real, who she's never met, who's a cop in another state and has never heard of her, or how he factors in to the story. Not sure how to tie up that loose end.

How about it was a wrong diagnosis and the patient gets sicker?

or one of the doctors friends or lovers or family could fall ill

The doctor's love interest could be upset about the amount of time he spends at work, or feel as though his medical practice is more important than their relationship, and then the doctor has to find a way to balance those things.

And a question: if the hallucinations are about someone who doesn't factor in to the story, why have them be about a real person?
If it's important that they be 'real', then you could have the cop be related to the patient in some way... a half-sibling that was put up for adoption etc. Or someone that had some life changing experience similar to something she experienced, perhaps as children? The cop and the patient were born at the same time? Or he had/has the disease the patient does?
