The doctor's love interest could be upset about the amount of time he spends at work, or feel as though his medical practice is more important than their relationship, and then the doctor has to find a way to balance those things.
And a question: if the hallucinations are about someone who doesn't factor in to the story, why have them be about a real person?
If it's important that they be 'real', then you could have the cop be related to the patient in some way... a half-sibling that was put up for adoption etc. Or someone that had some life changing experience similar to something she experienced, perhaps as children? The cop and the patient were born at the same time? Or he had/has the disease the patient does?
