In the movie North By Northwest, we have a case of mistaken identity, but the Cary Grant character is referred to by his true name in each character cue of the entire script.
True, but we know him as Roger Thornhill right from the start and he keeps insisting that he's Roger Thornhill all the way through.
But Cary Grant in Charade (1963 with Audrey Hepburn as 'Reggie' - great film) is a different thing.
He presents himself as one person, then claims to be another (and another, and another) In that script (it's available on the web) they do it like this:
